Liability Waiver

(For any and all of CSG Creations, LLC "scentedReys" workshops, candle making, or classes; and referred to as "We" or "us" throughout this document)

RELEASE/WAIVER: I hereby agree to indemnify and hold harmless CSG Creations, LLC and its employees against any and all claims for personal injuries or damages of any kind arising from participation in any candle making workshop, classes, and their resulting products.

PHOTO/VIDEO CONSENT: CSG Creations, LLC reserves the right to use photos, videos, and other media taken during classes, workshops, and other events used for publicity materials and social media. Registration in classes gives us permission to use those images without further consent or compensation.

NOT MEDICAL ADVICE: I hereby acknowledge that none of the materials, descriptions, or guidelines presented are intended to treat, prevent, or cure any disease or condition.

HAZARD ACKNOWLEDGEMENT: Due to the potentially dangerous nature of wax at high temperatures which may rise up to 200 degrees Fahrenheit and hot equipment including but not limited to metal wax melters and pouring pots, it is the responsibility of the participant to ensure the proper placement of their skill level. We cannot accept liability for any personal injury or damages that the participant may suffer related to or involving the premises of the event or participation in activities sponsored by us. Every effort is made by CSG Creations, LLC and its employees to ensure the participant's safety while at the event premises and during the course of the workshop.

I fully understand and acknowledge that the workshops and/or activities offered by CSG Creations, LLC may involve inherent risks, dangers and hazards which may result in injury or illness. I hereby agree to release, waive, discharge, hold harmless, defend and indemnify CSG Creations, LLC, its managers, members, and employees from any and all claims, actions or losses for bodily injury, property damage, wrongful death, or otherwise, which may arise from my participation in any workshops or classes, resulting products from workshops or classes, or from use of equipment provided by CSG Creations, LLC. I accept full liability for the products and/creations made by the participant in the workshop.
